- Serve as the primary contact for patients on the RPM365 platform.
- Assist with device setup and app connectivity.
- Engage with patients to ensure adherence to care plans.
- Schedule monthly (or more) check-ins and monitor patient data.
- Educate patients through shared resources.
- Notify healthcare providers of critical alerts.
- Attend virtual team meetings for updates.
- Active New York State nursing license.
- Knowledge of chronic conditions and related monitoring devices.
- Proficiency with technology, computer platforms, and smartphones.
- Strong verbal/written communication skills.
- Comfortable with patient outreach and engagement.
- Highly organized, detail-oriented, and team-focused.
- Independently motivated
- Comfortable spending the majority of your workday engaging with patients over the phone providing support, education and guidance.
- Experience in telehealth, remote patient monitoring, education, or customer service.
- Experience in weight management coaching
- Additional language skills are a plus.
- Salary: $45,000-$49,000 (DOE) + performance bonuses.
- Schedule: Mon-Fri, 9:00 AM – 5:00 PM, EST (Remote).
